CHECKLIST
FOR HIGH SCHOOL SENIORS
Get Ready for a Whole
New World...
Many high schools organize
a group registration for their students. If your school does not,
or if you are unable to attend with the group, call our office at
(805) 378-1461 and make a private appointment to register.
Follow this checklist
when you register at Moorpark College to attend summer and/or fall
classes. By providing us with the items on this list, we can get
you off to a successful start as a college student!
BRING
THE FOLLOWING ITEMS TO YOUR REGISTRATION APPOINTMENT:
*The results of your
completed Moorpark College Self-Placement Guides for Math and English
so we can help you register classes that are most suited to your
present skill levels.
*Your most recent I.E.P.
*If your diagnosis is
a LEARNING DISABILITY-- bring the scores of your most recent assessment
results. We need the raw scores (not just the Standard Scores and
Percentile Ranks) for both the cognitive tests and the achievement
tests. Often these scores are kept at your school's district office
instead of your high school site.
*If your diagnosis is
MEDICAL or a PSYCHOLOGICAL (ADD, ADHD, clinical -depression, bi-polar
disorder, etc) - bring a signed statement from your doctor, on letterhead,
stating your exact diagnosis and a list of any prescribed medications
and their recommended dosages.
*If you are under 18
--A statement signed by your parent or guardian that you have their
permission to register at Moorpark College.
*Your goals for attending
Moorpark College (an AA degree, transfer to a four-year college,
a vocational certificate of completion, exploring your options,
etc.).
*Ideas for the classes
you would like to take and the days and times you would like schedule.
Look at the catalog to learn about the classes …look at the schedule
to find out the days and times for each semester.
